Medical Records

We consider your medical information in this office a confidential and personal matter, therefore, you must sign a release of information before we will give any information from our medical records to any third party, including other physicians and insurance companies.

Remember, when you sign a release for an insurance company, you give them access to all information contained in our medical records unless you restrict the information to be released.
